Other than the gray screen, does it crash? freeze? what exactly is happening?
Have you contacted Softimage about this problem yet, they know more about their software than anyone here does.
This is all I could find on the XSI wiki about startup problems similar to yours, but you said it worked before, so I don't know...

Well, it doesn't freeze! The topbar works fine, only i can't see the viewports, and when inserting a primitive it gives me the red line in the pic.. I'll contact Softimage again.. Is there also other version of XSI which supports the exporter?

I'm happy to say this topic can be solved! I set my screen values to 1024xsomething and it all worked again! MandeRek is on one exporting-tour now

Bumping my own problem with the same problem actually.

The RunMeOnce.bat doens't seem to solve the problem, and when I type the line in cmd.exe it says the .dll isn't found and I need to make sure it's there, while it's there. My username is very long and has a space [ ] in it, so I might type that wrong? It's Firstname Surname, and I'm not sure how to type that in the regsvr32 line..

Anyhelp?

Edit: It says it can't find the binair module (last line)
I made a new account, with the name lorri, typed the line above and got the same error. See any problems?!

Stephen still works for Autodesk so you should be able to contact him through the support forums on the AREA or through eX-SI Support, he is also a member of SI-Community and XSIbase, you can get his email from his profile (login required to view profiles). He could probably help you but this problem really has nothing to do with XSI, you should search other technical support sites about problems registering dlls on Vista or Windows 7.

Don't ask how, but registering the dll on the other useraccount through the first useraccount made it work (on both user accounts )
